It is acquired by recording Chris, from within a locker, breaking into the Security Control room in the Administration Block.

- If Xbox One players choose to acquire this note, they will forfeit the Claustrophobe achievement, as it requires the player to finish the game without hiding under the beds or inside lockers.
- The note mentions Walker's patient file even if the player did not obtained the document.
