Nostophobia is a Routine Therapy program in The Outlast Trials. It consists of a variator that challenges Reagents to complete the Trials with a restricted loadout and limited resources.

Information

An aversion to the past, the antithesis of nostalgia. The only items in the map are batteries and bandages, and supply is limited.

This Routine Therapy removes certain gameplay mechanics and enemies, such as psychosis, traps (with the exception of Broken Glass and Noisemakers), secret rooms, throwable items, and the Specialist Ex-Pop, to allow players to experience the feeling of classic Outlast games. Reagents' night vision goggles have triple reduced battery lifespan, health bar reduced by one, cannot bring rigs and amps, and their only supply provided are syringes (Co-op), respawn pills (Solo), small batteries, and bandages. All Reagents are recommended to proceed with this Routine Therapy with every prescription to maximize efficiency.

This program has 3 Trials and 6 MK-Challenges.

Part II (July 8 - 15, 2025)

• This rotation was cancelled due to Routine Therapy tasks not counting. It instead used the same selection from Part I.

		Limited Inventory One slot of your inventory is disabled.
		Limited Items Amount of items found in the Trial is limited.
		High Enemy Density More enemies in the Trial.
		Limited Intel Assignment details are limited in the Trial.

		Bleeding Taking damage will cause bleeding. Use bandages to patch up.
		Limited Inventory II Two slots of your inventory are disabled.
		Limited Items II Amount of items found in the Trial is very limited.
		No Intel Assignment details are absent from the Trial.
		Increased Threat More and stronger enemies.

Educated: Collect 20 documents.
Claustrophobe: Finish a trial without using a hiding spot.
Energiser: Finish a trial without completely depleting the night vision's battery.
Proper Penance: Crouch walk for 2000 meters.
Heal the Sick: Use 25 bandages.
Slip and Slide: Slide for 50 meters.
Asahel: Finish a trial in under 8 minutes.
Sanctified: Finish 3 trials at standard difficulty.
Preacher: Finish 5 trials at Intensive difficulty.
Saint: Finish 3 trials at Psychosurgery difficulty.
Messiah: Finish a trial at Psychosurgery without reloading the night vision's battery.

Trivia

• Reagents cannot bring rigs and amps but can still use prescriptions. This is a reference to the first Outlast game where the first two protagonists could barely run out of stamina despite constantly running and vaulting, and additionally a reference to Outlast 2 where the protagonist has faster speed and a sliding ability.
• The Trials can only allow small batteries and bandages to spawn, which is also a reference to the previous Outlast games. However, the game still allows the respawn pills and syringes to appear.
• Reagents taking the bleeding effect on higher difficulties and requiring bandages to heal is a reference to the injury mechanics in Outlast 2.
• This Routine Therapy features tasks that reference the achievements from previous Outlast games.
• Reagents cannot be inflicted with psychosis from activating wrong objectives like valves in Grind the Bad Apples. This will also not penalize their grade.
• This is the first Routine Therapy program featured after the rename of Weekly Therapy program.
